Study "The Coimbra Protocol in MS"

The first study on the long-term safety and efficacy of ultra-high-dose vitamin D therapy according to the Coimbra Protocol was carried out until December 2025 at the NeuroCure Research Center of the Charité University Hospital in Berlin.

The observational study “The Coimbra Protocol in MS”, led by Prof. Friedemann Paul, enrolled 100 patients with relapsing-remitting multiple sclerosis who were at the start of their treatment with a certified protocol doctor.

The participants’ data were compared with similar cases from the Charité’s ongoing study register, in order to document efficacy compared with pharmacological baseline therapies or treatment-naïve courses.

This independent, donation-funded study came about on our initiative, as we — the non-profit Coimbraprotokoll UG — are committed to research and scientific documentation. In this way we help to ensure that the Coimbra Protocol becomes recognised and widely known as a therapy for autoimmune diseases.

We expect the study to be published during the course of 2026.

Current state of research

 According to the current state of research, impaired vitamin D utilisation and activation are attributable to single nucleotide polymorphisms (abbreviated SNP). This can lead to disturbances in the formation, activation or binding of vitamin D to the cell receptor. Since vitamin D is an important immune regulator, disturbances of the immune system follow from this.

The fundamentals of gene expression, or epigenetics, through vitamin D have been researched by, among others, Prof. Carsten Carlberg. In his work he has demonstrated that a part of the world’s population, because of their SNPs, needs much higher vitamin D blood levels in order to maintain healthy immune system function.

As early as 2002, Dr Coimbra put forward the theory of a “vitamin D utilisation impairment” and researched it in his practice and at the University of São Paulo.

In 2013, he and his team published a study on the use of ultra-high-dose vitamin D therapy according to the Coimbra Protocol in patients with vitiligo and psoriasis. In it, he explained the scientific basis of the Coimbra Protocol’s effect on the immune system. According to Dr Coimbra, this mechanism of action applies to all diseases at whose root lies a derailment of the immune system’s TH and Treg cells. Since this generally applies to autoimmune diseases, this means that ultra-high-dose vitamin D therapy according to the Coimbra Protocol is effective in all autoimmune diseases.
